"Certificado de aptitud profesional" (?) I'm thinking this is probably "Certificado de aptitud profesional", frequently abbreviated as "CAP". However, although I found numerous references from Spain & the EU, I couldn't track any down for Peru.

Though I don't know how to translate the term. It seems to be similar to the Spanish 'oposiciones' system.

If it is this, and it only exists in Peru, an alternative would be translating it as CAP Number, with an explanation in English.

neutral philgoddard : Maybe, but I'm not convinced. This is about resource allocation - it's not something you'd put on your credentials. It has to be some kind of registration or licence number.

I'm not familiar with how the system works in Peru, but maybe somebody who is will be able to confirm this either way. And I trust Liz's judgement regarding medical questions.
