Medical students are taught early on not use use phrases such as the source code. It is too vague. It may be true that the patient had a lumbar radiculopathy but what does it mean he "presented" with it? Did he present with pain? Did he have numbness and tingling? Did he have all three, or perhaps two of the three? Patients "present" with symptoms and "lumbar radiculopathy" is a diagnosis, not a symptom. So, a physician reading this medical shit is left with a vague idea of what is going on. You poor medical translators then fall into the trap of making assumptions - pain, numbness, tingling, all three? In some situations your assumptions could KILL someone. So your translation must replicate the error and be as vague as the original. Leave it to the medical professional to figure it out.
