**The term tension nose is known to many rhinoplastic surgeons, yet confusion exists as to its precise meaning. We define the tension deformity as excessive growth of the quadrilateral cartilage, resulting in a high nasal dorsum and anterior and sometimes inferior displacement of the nasal tip cartilages.**

Tension-Nose Deformity
A naturally-occurring cosmetic deformity characterized by dorsal overgrowth of the nasal septum. Hallmark features include "pollybeak" overgrowth deformity of the nasal bridge, overprojection of the nasal tip, and a wide nasal pedestal.

narrow nose syndrome
Síndrome de tensión nasal (nariz con giba)

Bilateral spreader grafts were used in cases of "narrow nose syndrome" (short nasal bones, long and weak upper lateral cartilages, thin skin) and in cases of disproportionate nose with narrow middle vault and bulbous tip.

On the other hand, a dynamic problem that involves the internal nasal valve usually involves a collapse of the lateral nasal sidewall due to a destabilization of the LLC or nasal septum.3 If these structures are disrupted and not refixated, they no longer have the stability to support the ULC, leading to collapse. Sheen22 observed that a patient with an overprojected nose, a narrow middle vault, and short nasal bones represents a “narrow nose syndrome.” This situation presents a higher risk for internal nasal valve narrowing after resection of the middle vault roof. This can result in an inferomedial collapse of the ULC into the airway.10 Spreader grafts are required in this setting.

Patients with short nasal bones, long upper lateral cartilages, and a projecting nose are at increased risk for collapse of the middle vault. Simple palpation of the nasal bone length can help predict patients who will be at higher risk of postoperative middle vault collapse. Sheen and Sheen3 referred to these patients as those with the “narrow nose syndrome.” When dorsal hump reduction is performed, there may be loss of medial support, and the upper lateral cartilages can collapse inferomedially (Fig. 5-2).5
