I would simply translate the entire sentence together "procedo a la siguiente inscripción" = I hereby make the following entry / I hereby record the following / I hereby add the following record.... something along those lines with the word "hereby" in it.....

I don't know if I'd use the past tense. I think with the word "hereby" the present would also work in this case... although the past could be another valid option.
