The Adams forward bend test should be performed after physical inspection of the spine. The patient is asked to bend forward at the waist with the knees straight and the arms passively hanging down. The examiner looks at the spine from behind and notes any thoracic or lumbar prominence indicating a rotational deformity of the spine.

Aufschlussreich ist auch der sogenannte Vorbeugetest oder Adams-Test. Dabei beugt der Betroffene bei gestreckten Beinen seinen Oberkörper um etwa 90 Grad nach vorne und lässt die Arme herunterhängen. Betrachtet man nun seinen Rücken von hinten, fällt bei einer Skoliose meist auf, dass eine Seite höher steht als die andere, dass sich eine Art Buckel (Rippenbuckel) bildet. Auch die Lendenmuskeln treten auf dieser Seite deutlicher hervor (Lendenwulst).
