That's what I mean In the sentence you give, it is clear that the expression needs to be analysed as I have suggested; on its own 'rétrograde à l'air' has no stand-alone meaning.

Parsing? I'm no medical expert, but surely you're parsing it wrongly? Isn't it rather a 'pyélographie rétrograde' followed by 'à l'air'?

Tony is right - you've parsed the sentence wrongly. Also, I think the French syntax is wrong. It appears to be two phrases, "embolie à l'air" and "pyelographie rétrograde", so "air embolism" and "retograde pyelography".

In fact, I think the two cases described in my reference may be those referred to in the asker's text.
