La massima dose saggiata "in vivo" è stata 300 mg/kg e.v. e solo a questo dosaggio si sono evidenziati alcuni effeti degni di segnalazione. Questi possono essere cosi riassunti:
- abbassamento significativo e duraturo della pressione sisto-diastolica, non associato a variazione nell'indice contrattile. Il fenomeno che non sembra influenzare la meccanica contrattile cardiaca (tra l'altro in accordo con i risultati della sperimentazione in vitro sull' atrio isolato di cavia) potrebbe essere attribuito ad un efetto del farmaco sulla muscolatura liscia vascolare;
-riduzione delle contrazioni della membrana nittitante (circa 30%), associata a deboli incrementi della risposta pressoria a Noradrenalina ed Adrenalina dose-dipendenti. Negli stessi animali utilizzati per lo studio della membrana nittitante, l'incremento della pressione conseguente alla occlusione delle carotidi, veniva in gran parte attenuato da trattamento con xxx (gatto anestetizzato); questi risultati sembrano indicare per questa unica dose del farmaco, un debole effetto ganglioplegico in accordo anche alla riduzione della risposta pressoria al DMPP. Inoltre l'inibizione del riflesso carotideo lascia trasparire un effetto di xxx sui meccanismi centrali dell' attivazione simpatica anche se tali effetti possono essere complicati dalla contempoanea interferenza sia sulle stazioni gangliari di tali vie efferenti sia di una componente inibitoria diretta sulla muscolatura liscia vascolare.
Translation - English
The maximum dose tested "in vivo" was 300 mg/kg i.v., and only at this dose were any noteworthy effects observed. These may be summarised as follows:
- a significant and prolonged decrease in systolic and diastolic blood pressure not accompanied by a change in contractile index. This response did not seem to affect cardiac contractile mechanics (and thus, among other things, agrees with the results of "in vitro" experiments on the isolated guinea pig atrium) and could be attributed to an effect of the medicinal product on the vascular smooth musculature;
- a reduction of nictating membrane contractions (by about 30%) accompanied by a slight dose-dependent increase in the pressor response to noradrenaline and adrenaline. In the same animals used for the nictating membrane study, the increase in pressure following carotid occlusion was largely attenuated by treatment with xxx (anesthetised cat). These results seem to suggest that there is a weak ganglioplegic effect with this single dose of the medicinal product, which also agrees with the reduction of the pressor response to DMPP. In addition, the inhibition of the carotid reflex reveals that xxx has an effect on the central mechanism of sympathetic activation even though this effect may be complicated by concomitant interference with the ganglia of these efferent pathways or by an element of direct inhibition of the vascular smooth musculature;

Le conoscenze scientifiche degli ultimi anni hanno aggiunto sempre più robuste evidenze che il neonato, soprattutto se pretermine, si dimostra più sensibile allo stimolo nocicettivo a cui risponde con una reazione fisiologica e comportamentale ben evidente e con una cascata ormonale e metabolica, il cui reiterarsi può comportare effetti destabilizzanti a breve e medio termine. Questa fragilità e vulnerabilità allo stimolo doloroso soprattutto del neonato pretermine è spiegabile dal punto di vista neurofisiologico con il fatto che, a scapito di un apparato sensoriale per la nocicezione ben sviluppato a partire dal secondo trimestre di gravidanza, i sistemi discendenti inibitori e i loro neurotrasmettitori preposti alla modulazione delle afferenze nocicettive sono deficitari ed immaturi fino anche a dopo la nascita a termine (3). Inoltre esistono sempre maggiori segnalazioni degli affetti a lungo termine di una esposizione precoce e ripetitiva a stimolazioni nocicettive in un momento cosi fondamentale per lo sviluppo del sistema nervoso. Sono state segnalate modificazioni comportamentali a lungo termine e riduzioni di volume delle aree cerebrali sensitive in ex pretermine ricoverati in TIN, simili a quelle evidenziate nel modello sperimentale di ratto di pari età gestazionale, sottoposto a stimuli doloroso precoci e ripetitivi (4-5).
Nonostane queste evidenze a tuttoggi il trattamento del dolore da procedura rimane limitato nel neonato. Da recenti dati, un neonato ricoverato in TIN viene sottoposto in media a 14 procedure dolorose al giorno (6). Inoltre sempre più frequentemente nelle nostre TIN vengono eseguite manovre chirurgiche come il posizionamento di un catetere venoso centrale chirurgico, la legature del PDA, la laser o crioterapia per ROP.

Scientific research in recent years has supplied increasingly firm evidence that neonates, especially when preterm, are more sensitive to nociceptive stimuli. Neonates are capable of mounting robust physiological, behavioural, hormonal and metabolic responses to such stimuli, which can have adverse effects in the short and medium-term. The reason why preterm neonates in particular are so vulnerable to pain is because of the neurophysiological factor that, although nociceptor and sensory neuron function is well developed from the second trimester of pregnancy, the descending inhibitory systems and their neurotransmitters which modulate nociceptive afferents are immature and ineffective until and even after term birth (3). Furthermore, there are an increasing amount of reports on the long-term effects of premature and repetitive exposure to painful stimuli during a period which is fundamental for the development of the nervous system. Long-term behavioural changes and a reduced volume of the sensory areas of the brain have been reported in preterm infants in NICUs. This is consistent with laboratory studies on rats of the same gestational age exposed to repetitive, painful stimuli early in development (4-5).
Despite this evidence, treatment for neonates undergoing painful procedures is still limited. According to recent reports, neonates in NICUs experience an average of 14 painful procedures a day (6). Moreover, surgical procedures such as central venous catheter insertion, PDA ligation, and laser or cryotherapy for ROP are regularly performed in Italian NICUs.

Gli interferoni esplicano importanti effetti immunomodulanti, antivirali ed antiproliferativi. Le differenze nel meccanismo di azione tra interferone alfa e beta sono responsabili del diverso effetto immunomodulante delle due molecole. Mentre l’interferone alfa aumenta la produzione di interferone gamma potenziando l’espressione degli antigeni MHC di classe II, l’interferone beta esercita un effetto opposto riducendo la produzione di interferone gamma, l’espressione di antigeni MHC di classe II ed attenuando le risposte autoimmuni. Ulteriori studi condotti su modelli animali hanno dimostrato che l’interferone beta inibisce la produzione di TNF-alfa da parte dei polimorfonucleati periferici.
Per quanto riguarda gli effetti antivirali, gli interferoni inducono la produzione di più di 20 proteine responsabili dell’attivazione di meccanismi antivirali all’interno della cellula. Queste proteine inibiscono la replicazione del DNA e l’assemblaggio dei virus. Gli stessi meccanismi antivirali sono responsabili anche degli effetti antiproliferativi. Complessivamente gli interferoni rallentano il ciclo replicativo cellulare, allungando l’intero ciclo senza agire su una fase specifica.



Translation - English
Interferons have important immunomodulant, antiviral and antiproliferative effects. The differences between the mechanisms of action of alpha and beta interferons are responsible for the differences between the immunomodulatory effects of the two substances. Interferon alpha increases the production of interferon gamma by potentiating the expression of class II MHC antigens, however, interferon beta has the opposite effect by reducing the production of interferon gamma and the expression of class II MHC antigens and attenuating the autoimmune response. Later studies conducted on animals have demonstrated that interferon beta inhibits the production of TNF-alpha by peripheral polymorphonuclear cells.
With regard to the antiviral effects, interferons induce the production of more than 20 proteins responsible for the activation of antiviral mechanisms within the cell. These proteins inhibit the replication of DNA and assembly of viruses. The same antiviral mechanisms are also responsible for the antiproliferative effects. In all, interferons slow down the cell replication cycle by prolonging the entire cycle without acting on a specific phase.
