Source text - English Historically, the procedures making up the ocular health examination (the external and internal examinations) have been completed as a part of the preliminary examination. However, with indirect ophthalmoscopy under dilation now a routine optometric procedure, the internal examination is normally postponed until after the completion of the refraction and the binocular vision examination. Additional procedures—not always performed, including gonioscopy, biomicroscopic examination of the posterior segment under dilation, and photodocumentation—may also be done at the completion of the examination.
But because of the importance of establishing the clarity of the lens and the macular area before beginning objective and subjective refraction, it is strongly recommended that biomicroscopy and direct ophthalmoscopy (without dilation) be done in the latter part of the preliminary examination.
THE USE OF DIAGNOSTIC PHARMACEUTICAL AGENTS
Three classes of diagnostic pharmaceutical agents are useful in the everyday practice of optometry:
• Topical anesthetics are used to anesthetize the cornea prior to tonometry gonioscopy.
• Mydriatics are used to dilate the pupil prior to the use of such procedures as binocular indirect ophthalmoscopy and biomicroscopy involving the posterior segment of the eye—with the aid of a Goldmann fundus lens or a 78.00D or 90.00D condensing lens—and fundus photography.
• Cycloplegics are useful in both objective and subjective refraction of patients having (or suspected of having) latent hyperopia or ciliary spasm. (Chapter 9 discusses cycloplegic agents and cycloplegic refraction.)

Ponieważ jednak oftalmoskopia pośrednia po rozszerzeniu źrenic jest aktualnie rutynowym badaniem optometrycznym, badanie struktur wewnętrznych zostawia się zwykle na koniec, po zakończeniu badania refrakcji i widzenia obuocznego. STOSOWANIE LEKÓW W CELACH DIAGNOSTYCZNYCH
W codziennej praktyce optometrycznej do celów diagnostycznych zastosowanie znajdują trzy grupy leków:
• Leki miejscowo znieczulające stosowane są do znieczulania rogówki przed wykonaniem tonometrii i gonioskopii.
• Środki rozszerzające źrenice stosuje się do rozszerzania źrenic przed takimi badaniami, jak oftalmoskopia obuoczna pośrednia i biomikroskopowa ocena tylnego odcinka oka – z pomocą soczewki Goldmana lub soczewek skupiających o mocy 78.00D lub 90.00D — oraz wykonaniem fotografii dna oka.
• Środki porażające akomodację są wykorzystywane do obiektywnego i subiektywnego badania refrakcji u pacjentów z rozpoznaną (lub podejrzewaną) utajoną nadwzrocznością bądź skurczem mięśnia rzęskowego. (Rozdział 9 omawia środki porażające akomodację oraz badanie refrakcji po porażeniu akomodacji.)
