I think it could also mean that some registration fee that the patient paid got forfeited because he/she didn't have the money to get further medical attention. In the Mao years it probably ran for 50 cents to one yuan

neutral Denyce Seow : In Singapore, we do say 'si3 qiao4 qiao4' for 'die', but I think 'escape' makes more sense in this sentence.
