It is quite clear that the antibiotics, or the coagulation factor8 (that in case of a hemophilia patient) are employed to prevent trouble during the surgery, "pod clonou" being a convenient brief term to for it.
So - is there, or else,
what would you suggest as a good English equivalent for this Czech / Slovak medical idiom?

"coverage" is commonly used in association with ATBs -- e.g. "covered by antibiotic prophylaxis" or "under antibiotic coverage".

I'd stick to Martin's suggestion for Factor VIII.
Example sentence:

... high-risk patients account for a very small proportion of those previously covered by antibiotic prophylaxis, thereby reducing the number exposed to any possible harmful adverse effects.

under/with prophylactic antibiotic (pre)treatment

ale osobně bych to celé vypustil - v angličtině krátký slangový výraz zřejmě není, takže je ten dovětek zcela zbytečný.
